Interested and eligible candidates can apply offline by sending the properly filled out official application form in a properly sealed envelope by superscribing the word “APPLICATION FOR THE POST OF __________” on the top of envelope by using the Ordinary Post/ Registered Post/ Speed Post to “36 Field Ammunition Depot, Pin-900484″ with in 21 days from the date of publication of the official advertisement and 28 days for the candidates belonging to Assam, Meghalaya, Arunachal Pradesh, Mizoram, Sikkim, Laddakh Sub Division of Jammu and Kashmir, Lahaul & Spiti District of Pangi Sub Division of Chamba district of Himachal Pradesh, Andaman & Nicobar Island & Lakshadeep.
The photocopy of the following documents/certificates to be attached along with application duly self-attested:-
1. Two passport size latest photographs duly self attested, one on right corner of application and one on Acknowledgement card.
2. Attested copies of the following certificates will also be submitted with application –
- Education Qualification Certificate
- Date of Birth Certificate
- Caste Certificate where applicable
- Discharge certificate for Ex-Serviceman or NOC from the competent authority for serving Defence Personnel who are completing the prescribed period of Army services within a year from the last date for receiving application
3. Medical certificate (format attached) issued by Central/ State Government medical board consisting of at least three members out of which at least one shall be a specialist in the particular field for assessing locomotor/ cerebral/ visual/ hearing disability and certifying the percentage of disability (i.e. 40% and above).
4. Self addressed envelope affixing postal stamps of Rs.25/-.
NOTE: Central Government Civilian Employees must furnish “No Objection Certificate” from their employer/office else their candidature will be cancelled.
